Key Responsibilities:
  • Assess client needs and develop personalized care plans
  • Deliver nursing care and services in accordance with federal, state, and local laws and regulations
  • Collaborate with regional directors, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to clients and their families on health-related topics and disease management
  • Document client progress and maintain accurate records
