Community Based Case Manager

Clarvida - Idaho is seeking a skilled Community Based Case Manager to join our team. As a key member of our community-based services team, you will work with adolescent through adult clients who have behavioral issues, emotional disturbance, or severe and persistent mental health diagnoses. Your primary responsibility will be to provide case management services, including skill building, gaining access to community resources, and providing coping skills to foster independence and improve their quality of life.
Key Responsibilities:
• Interface routinely with Lead IOP Clinician, Lead Supervisor, IOP staff, clients, and other key personnel as identified essential to client care.
• Complete required initial and ongoing trainings, including CEU's related to specific job duties, one of which must be ethics training annually.
• Attend weekly IOP Staff Meeting.
• Complete all progress notes related to IOP clients within 24 hours of date of service.
• Adhere to IOP schedule to ensure services are delivered a minimum of 3 days per week, maintaining at least nine (9) hours of service for adults and at least six (6) hours of service for adolescents. (Combined community rehabilitation services, individual therapy, group therapy)
• Provides service delivery in accordance with the objectives specified/authorized in the individualized treatment plan and in accordance with payer standards.

Requirements:
• Bachelor's degree: Human Service field a plus (Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, Physical Therapy, Occupational Therapy, Therapeutic Recreation, Speech Language Pathology, Behavioral Sciences, Art Therapy, etc)
• Ability to pass background checks, including Enhanced Fingerprinting with ID DHW.
• Valid driver's license with reliable transportation and proof of car insurance
• Experience utilizing electronic health records system a plus
