Job Summary

We are seeking a highly skilled Stroke Center Advanced Practice Nurse to join our team at Hackensack Meridian Health. As a key member of our stroke care team, you will provide exceptional patient care and contribute to the development of evidence-based practice protocols.

Responsibilities
  • Provide medical assessments, treatments, and clearances for patients, employees, and external customers, adhering to regulatory standards and company policies.
  • Deliver high-quality clinical care, serving as a customer advocate and role model, and ensuring accurate documentation of patient care.
  • Develop and implement individualized plans of care, incorporating current protocols and standards of care, and promoting effective recovery from on-the-job injuries or illnesses.
  • Facilitate patient education and support programs, and evaluate current protocols and standards of care to translate best practices into clinical care.
  • Apply evidence-based practices to achieve positive patient outcomes, and provide medication safely and effectively.
  • Prescribe medications and recommend over-the-counter medications as appropriate, and initiate laboratory tests, immunizations, and diagnostic tests/procedures as needed.
  • Collaborate with physicians and consult with other healthcare team members to achieve results, and demonstrate effective communication skills to achieve interdisciplinary outcomes.
  • Provide accurate, systematic, and organized documentation of patient care, and participate in continuing education and maintain documentation of personal development.
  • Develop a personal plan of development in area of clinical expertise, and confer with leaders on goals and objectives.
  • Provide culturally diverse and educationally appropriate education in relation to patient care/needs, and serve as a clinical resource to nursing staff, patients, and other multidisciplinary team members.
  • Participate in policy/procedure reviews for updating with current evidence-based and value-based practices, and act as a resource to staff for addressing clinical, process, and system opportunities for improvement.
  • Promote a collaborative work environment that supports the organizational mission, behavior expectations, and values, and promote innovation within clinical practice by seeking opportunities to bring new clinical knowledge into practice.
  • Identify ethical care issues and coordinate interdisciplinary collaborative meetings to resolve those issues, and float to various clinic locations as needed.
Qualifications
  • Graduate of an NLN/AACN accredited program in nursing, with a Master's Degree.
  • Maintains clinical competencies through participating in continuing educational programs and clinical practice as mandated by the Board of Nursing and national certification body.
  • Experience in relevant patient care setting or as regulations dictate, with a strong organizational and computer skills, effective verbal and written communication skills, emotional intelligence, critical thinking, and effective interpersonal skills.
  • Ability to adapt to multiple and changing priorities, with a valid driver's license, NJ State Professional Registered Nurse License, Advanced Practice Nurse Certification, and AHA Basic Health Care Life Support HCP Certification.
