About the Role:
We are seeking a highly motivated Nurse Practitioner to join our team at Healthcare Talent Support. As a key member of our care team, you will be responsible for leading the team-based care for individuals with serious mental illness.
Key Responsibilities:
- Perform a comprehensive clinical assessment, including gathering information from outside sources and assessing both behavioral and physical health conditions.
- Conduct visits in the field or in an individual's home when needed, and make individualized recommendations and/or referrals for programs/care provided by firsthand or outside providers.
- Train and advise the care team, and support the care team on care coordination.
- Perform medication reconciliation post-discharge and as needed, and communicate and coordinate with individuals care providers.
Requirements and Qualifications:
- Empathy, compassion, and approachability.
- Excellent listening, communication, and interpersonal skills.
- Desire to help people in need and support their path to wellness.
- Cultural humility, knowledge of trauma informed care, and social determinants of health.
- Desire to perform data driven care.
- 5+ years clinical care as a Nurse Practitioner, ideally in a primary care setting.
