Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at Phoenix Children's. As a key member of our interdisciplinary team, you will provide clinical assessment, therapeutic interventions, and care coordination to patients and families.

Key Responsibilities
  • Clinical Assessment and Intervention

Conduct comprehensive bio-psychosocial assessments to identify therapeutic goals and objectives for patients and families. Develop and implement individualized treatment plans to address mental health, behavioral, and emotional needs.

Care Coordination

Collaborate with interdisciplinary teams to coordinate effective treatment interventions, develop comprehensive treatment plans, and design therapeutic interventions for patients. Cultivate and maintain effective communication with medical staff, nursing staff, case managers, peers, and families to facilitate care coordination and continuity of care.

Documentation and Record-Keeping

Complete clinical record documentation in a timely manner, providing information to coordinate care, confirm demographic information, and provide recommendations to assist colleagues and medical teams with disposition planning.

Education and Professional Development

Maintain professional competency by participating in educational programs, reviewing relevant professional publications, and participating in community-based committees related to service line responsibilities. Stay current with community and other resources networks to meet patient and family needs.

Requirements

To be successful in this role, you will need:

  • Master's degree in Social Work (MSW) or related field
  • Experience in pediatric social work or related field
  • Strong clinical assessment and intervention skills
  • Excellent communication and collaboration skills
  • Ability to work effectively in an interdisciplinary team environment
