University of New Mexico is seeking a skilled Registered Nurse - Case Manager to join our team. As a key member of our healthcare team, you will be responsible for coordinating patient care and ensuring the delivery of high-quality, patient-centered services.
Key Responsibilities:- Develop and implement individualized care plans for patients with complex medical needs.
- Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Current RN licensure in New Mexico or eligibility for licensure by reciprocal agreement.
- 2 years of experience in a healthcare setting, preferably in case management or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
