Position Summary

Job Title: Healthcare Liaison

Primary Responsibilities: The Healthcare Liaison will play a vital role within the Patient Support Services team, focusing on assisting patients in navigating their healthcare journey. This position involves outreach to individuals primarily located in designated communities, with travel as a necessary component of the role.

Work Schedule: Full-time position, typically Monday through Friday, with standard working hours.

ROLE DESCRIPTION: The Healthcare Liaison will collaborate closely with a multidisciplinary team, including Care Managers and Social Workers, to facilitate the development and execution of personalized care plans. The Liaison will serve as a key advocate for patients, ensuring they receive the necessary support to meet their health objectives. Responsibilities include:
  • Engaging with patients to provide guidance and support in their healthcare decisions.
  • Coordinating with various community resources and healthcare providers to enhance patient care.
  • Assisting patients in understanding and adhering to their treatment plans.
  • Documenting patient interactions and maintaining accurate records in compliance with organizational standards.
  • Participating in team meetings to discuss patient progress and strategies for improvement.
KEY FUNCTIONS:
To excel in this role, candidates should possess the following abilities:
  • Ability to work under the direction of a care team to implement care management strategies.
  • Experience in coordinating social services and healthcare resources.
  • Strong communication skills, both verbal and written, to effectively interact with patients and team members.
  • Capacity to manage multiple priorities and adapt to changing circumstances.
  • Valid driver's license and access to a reliable vehicle for travel to patient locations.
PREFERRED QUALIFICATIONS:
  • Experience in a healthcare or social services environment.
  • Knowledge of community resources and support services.
  • Ability to work with diverse populations and understand their unique needs.
WORK ENVIRONMENT:
The position requires occasional travel to various locations, including patient homes and community facilities. The role involves a combination of office work and field activities, with a focus on providing compassionate support to patients throughout their healthcare experience.
