Job Title: Clinical Care Coordinator - Patient Centered Approach

We are seeking a dedicated and compassionate Clinical Care Coordinator to join our healthcare team at Community Health System. This role is ideal for a registered nurse with excellent communication and interpersonal skills.

About the Role:
  • This position involves assessing patient needs, developing individualized care plans, and providing high-quality nursing care in accordance with established standards and protocols.
  • The successful candidate will be responsible for communicating effectively with patients, families, and healthcare teams to ensure seamless care delivery.
  • This role requires strong organizational and time management skills to prioritize tasks and manage multiple responsibilities simultaneously.
Key Responsibilities:
  • Assess patient needs through thorough evaluations and identify areas requiring additional support or intervention.
  • Develop, implement, and evaluate individualized care plans that address patient-specific needs and goals.
  • Collaborate with multidisciplinary teams to coordinate care, share knowledge, and improve patient outcomes.
Requirements:
  • Graduate of an accredited School of Nursing with a current RN license in the State of Alabama.
  • Demonstrated expertise in nursing practices, including patient assessment, care planning, and communication.
  • Excellent interpersonal and teamwork skills, with the ability to work effectively in a fast-paced environment.
