ResponsibilitiesPhysical Therapist- Conduct thorough assessments of patients' physical abilities, medical histories, and home environments to develop personalized treatment plans.
- Provide skilled physical therapy services including therapeutic exercises, manual therapy techniques, gait training, and balance training.
- Educate patients and their caregivers on proper techniques for exercises and activities to promote recovery and maintain functional independence within their home environment.
- Monitor and document patients' progress, modifying treatment plans as necessary to achieve therapeutic goals and ensure patient safety.
- Collaborate closely with physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
- Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
- Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.
- Assist in conducting assessments of patients' physical abilities, medical histories, and home environments under the direction of a licensed Physical Therapist.
- Implement prescribed physical therapy treatment plans, including therapeutic exercises, manual therapy techniques, gait training, and balance exercises.
- Educate patients and their caregivers on exercises and activities prescribed by the supervising Physical Therapist to promote recovery and maintain functional independence within their home environment.
- Assist in monitoring and documenting patients' progress, reporting observations to the supervising Physical Therapist and modifying treatment plans as necessary.
- Collaborate closely with the supervising Physical Therapist, physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure continuity of care.
- Maintain accurate and timely documentation of patient evaluations, treatment sessions, and progress notes in accordance with regulatory and organizational standards.
- Adhere to ethical and professional standards of practice, respecting patient confidentiality and safety protocols specific to the Merrimack Valley and North Shore communities.
- Degree in Physical Therapy (for PT) or Associate's degree from an accredited Physical Therapist Assistant program (for PTA).
- Current state licensure or eligibility for licensure as a Physical Therapist or Physical Therapist Assistant in Massachusetts.
- CPR certification.
- Must pass a background check.
- Clinical experience in a healthcare setting, with preference given to those with experience in home care or community-based settings.
- Strong interpersonal skills, with the ability to communicate effectively with patients, caregivers, and healthcare team members.
- Ability to work independently and manage time effectively in a home care environment.
- Compassionate and patient-centered approach to care, demonstrating sensitivity to cultural diversity and the unique needs of the Merrimack Valley and North Shore residents.
- Knowledge of Medicare and Medicaid regulations related to home health services in Massachusetts.
