Physical Therapist

We are seeking a skilled Physical Therapist to join our team at the Rehab/Wellness Center on the West Coast of Oregon. As a member of our 17-strong team of therapists, you will be part of a supportive and collaborative environment that values integrity and best practice patient care.
Our center delivers rehabilitation services to a diverse outpatient caseload, and we also provide care to inpatients in a 21-bed Critical Access Hospital. We have a dynamic interdisciplinary team that works together to provide comprehensive care.
Our state-of-the-art gym is equipped with the latest technology, including electric mat tables, powered parallel bars, and Sci-fit bikes. We also have a vestibular program utilizing the RealEyes xDVR Video Goggles, and each therapist has their own computer-on-wheels to document at point of service.
Responsibilities- Evaluate patients and establish treatment plans and goals, using data from various sources, such as patient records, interviews, observation, and team members.
- Provide patient treatment according to the plan of care and modify the plan to meet patient needs, and update the plan as necessary.
- Complete documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
- Collaborate with other healthcare professionals as necessary to ensure smooth continuous care of patients. May act as a liaison with nursing, medical, and other therapy staffs to facilitate problem-solving and coordination of other services and act as an educational resource.
- May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.
- Recommends and participates in development of new services and programs, quality improvement, and monitoring activities.
- Bachelor's, Master's, or Doctorate Degree Required: Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted)
- Required: Physical Therapist license State of Oregon
- Required Upon Hire: Basic Life Support issued through the American Heart Association or American Red Cross
