At Open Sky Community Services, we are seeking a highly skilled and compassionate Outreach Case Manager to join our team. As a key member of our community support team, you will play a vital role in providing in-home supports and services to individuals with developmental disabilities, enabling them to live independently and participate fully in their communities.

Key Responsibilities
  • Develop and implement individualized service plans to support individuals in achieving their goals and aspirations.
  • Link individuals to community resources and services, and assist them in managing and coordinating these resources.
  • Facilitate the development of valued community roles and relationships, promoting social inclusion and participation.
  • Implement medical and medication protocols, and assist individuals in accessing necessary transportation.
  • Provide crisis intervention and participate in on-call rotations to ensure 24/7 support.
Requirements
  • High school diploma or equivalent; alternative education/experience considered.
  • Valid driver's license and acceptable driving record.
  • Reliability, willingness to learn, and openness to new opportunities.
