Key Responsibilities- Develop and implement individualized service plans to support individuals in achieving their goals and aspirations.
- Link individuals to community resources and services, and assist them in managing and coordinating these resources.
- Facilitate the development of valued community roles and relationships, promoting social inclusion and participation.
- Implement medical and medication protocols, and assist individuals in accessing necessary transportation.
- Provide crisis intervention and participate in on-call rotations to ensure 24/7 support.
- High school diploma or equivalent; alternative education/experience considered.
- Valid driver's license and acceptable driving record.
- Reliability, willingness to learn, and openness to new opportunities.
- Competitive hourly wage of $18.75.
