Job Summary

Hamilton Community Health Network Inc is seeking a highly skilled Optometrist to join our team. As a member of our Medical team, this position is responsible for providing optometry services to patients.

Key Responsibilities
  • Conduct eye exams using optometric equipment and general diagnostic interpretation.
  • Diagnose and treat ocular disease and defects of the eye.
  • Monitor eye health for people with certain medical conditions.
  • Prescribe medications and vision aids.
  • Perform pre and post operation checks.
  • Recommend patient follow-up care options.
  • Answer patient questions and provide advice on vision and optical issues.
  • Direct optometry staff and assist in staff development.
  • Provide patients with education and ensure they receive relevant eye care materials.
  • Refer patients to specialists when necessary.
  • Ensure continuum of care by using HCHN supported laboratory services and pharmacy services.
  • Complete all paperwork requirements for encounters and medical records compliance.
  • Ensure patient charts are completed within 24 hours and are comprehensive and legible.
  • Adhere to contract specifics related to patient volume and assist with clinic flow.
  • Perform eye examinations and procedures as assigned.
  • Prescribe medications, contact lenses, and eye glasses as assigned.
  • Advise patients on good eye care practices and eye exercises.
  • Interpret and present clinical data to the CEO, management team, and Board of Directors.
  • Develop, implement, and monitor the HCHN CQI program related to clinic patient care.
  • Participate in assigned meetings, including provider meetings, staff meetings, and Peer Review meetings.
  • Perform and accept other responsibilities as assigned.
Qualifications
  • Valid and active State of Michigan license as a Doctor of Optometry (OD).
  • Board Certified in Optometry.
  • Comprehensive knowledge of eye care delivery and organizational management.
Work Environment

The work environment is a clinical office space. The employee may be exposed to communicable diseases and may be required to travel between sites using their own vehicle.
