Care Management Coordinator Specialist

We are seeking a highly skilled Care Management Coordinator to join our team at MJHS. As a key member of our healthcare team, you will play a vital role in ensuring high-quality, cost-effective care and services for our Elderplan members.
Responsibilities- Support professional Care Management and/or Clinical Service activities to deliver exceptional care coordination for our Medicare, Medicaid, and/or dual product members.
- Collaborate with healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of our members.
- Ensure compliance with all departmental and regulatory requirements, including utilization management and case management.
- High School Diploma or equivalent; college degree preferred.
- One year prior managed care experience required.
- Prior experience in a healthcare setting preferred.
- Familiarity with utilization management/case management.
