Key Responsibilities- Case Management: Provide comprehensive case management support to patients, including clinical intervention, care plan development, and follow-up support.
- Clinical Leadership: Serve as a clinical leader and mentor to Home Health Aides, ensuring that every patient receives top-quality support and care.
- Documentation: Extensively document every patient visit in the Electronic Medical Record (EMR), ensuring timely and accurate charting for clinical team reference and auditing purposes.
- Training and Supervision: Orient and supervise Home Health Aides in the field, ensuring that they have the necessary skills and knowledge to provide high-quality care.
- Education: Current license to practice as a Registered Nurse in New York State.
- Experience: Minimum of two years of experience as an RN in a home care setting.
- Skills: Strong clinical skills, excellent communication and leadership abilities, and the ability to work independently in a fast-paced environment.
