Job Summary

We are seeking a highly skilled and compassionate Community Mental Health Nurse Care Manager to join our team at Project HOME. As a key member of our healthcare team, you will play a vital role in providing exceptional care to individuals with serious mental illness.

Key Responsibilities
  • Coordinate care with high-risk patients, behavioral health providers, and other specialists to ensure comprehensive and effective treatment.
  • Manage prior authorizations, scheduling, and administration of antipsychotic injections for patients at SKWC.
  • Collaborate with the Mobile Nursing Team and psychiatric care providers to provide care management and scheduling for patients living in Project Home Residences.
  • Provide nursing assessments and triage to walk-in patients and handle incoming calls from high-risk patients according to the Stephen Klein Wellness Center triage protocol.
  • Develop and maintain activities to achieve Patient Centered Medical Home as required.
  • Document clinical and care management activity in NextGen.
Requirements
  • Bachelor of Science in Nursing (BSN)
  • 3 years of prior experience as a nurse care manager
  • Active RN license in Pennsylvania
  • BLS certification
  • Preferred qualifications include one or more years of experience working as an RN with the target population, prior experience delivering nurse care management services to patients with complex care concerns, and proficiency in Microsoft Outlook.
