We are seeking a highly skilled LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing high-quality care and services to our residents, ensuring their safety and well-being.
Key Responsibilities- Administer medications and treatments in a safe and organized manner, adhering to federal, state, and local laws and regulations.
- Document medication administration and resident care activities, maintaining accurate and up-to-date records.
- Conduct Service and Health Updates, and participate in the development of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
- Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure seamless care delivery and resident satisfaction.
- Participate in training and education programs to maintain certifications and stay up-to-date on industry best practices.
- LPN, LVN, or state/province specific licensed nurse credential.
- CPR and First Aid certifications.
- Ability to make choices and decisions in the best interest of residents.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
