Home Health Nurse

Interim HealthCare Inc is seeking a skilled Home Health Nurse to provide high-quality care to patients in their homes. As a Home Health Nurse, you will work with a multidisciplinary team to assess patients, develop care plans, and provide education to patients and their families.
Responsibilities:- Conduct home visits to assess patients' needs and develop care plans
- Provide education to patients and their families on disease management, medication, and home safety
- Collaborate with physicians and other healthcare professionals to coordinate patient care
- Document patient progress and communicate with the healthcare team
- Graduate of an accredited nursing program
- Active, unrestricted nursing license in the State of South Carolina or compact multi-state license
- Minimum of one year of nursing experience, ideally in home healthcare or a related field
