We are seeking a highly skilled Licensed Practical Nurse to join our team at HealthEcareers - Client. Key Responsibilities- Assume charge and direction of the floor, working closely with the Director of Resident Services to ensure quality resident care.
- Create nurse aide assignments as needed to meet resident needs.
- Notify the Director of Resident Services of any changes in resident condition or issues with staff members.
- Administer medications and treatments according to established policies and procedures.
- Keep the medicine cart locked at all times when out of view.
- Update resident support plans daily as needed.
- Assess residents and follow established policies and procedures.
- Assist with resident ADLs as necessary.
- Assist with resident meals and fluid intake to ensure proper nutrition.
- Accompany physicians on resident visits to ensure continuity of care.
- Transcribe physician orders in a timely manner.
- Observe nursing care and visit residents throughout the shift to ensure care is carried out as directed.
- Participate in receiving report from the off-going shift and giving report to the on-coming shift to ensure continuity of care.
- Monitor care provided by Nurse Aides.
- Complete resident incident/accident reports in a timely manner.
- Collect samples from residents for testing as needed.
- Communicate information as necessary with families, residents, staff, etc.
- Ensure adequate supplies are available on the floor for all disciplines.
- Participate in staff meetings to enhance communication between all disciplines.
- Participate in problem resolution regarding employees and floor issues.
- Participate in orientation and training of personnel.
- Maintain a clean, safe, and orderly environment for residents, staff, and visitors.
- Accurately document all care provided to residents during the shift and before the end of the shift.
- Chart medications immediately after administration, per policy.
- Provide input to the Director of Resident Services concerning nurse aide performance.
- Maintain confidentiality.
- Attend and participate in staff meetings, in-services, mandatory trainings/in-services, and continuing education programs.
- Function within the limits of the Pennsylvania State Professional Standards Title 49 Chapter and in accordance with the scope of the practice permitted with licensure.
- Perform other duties as assigned.
- Graduated from an accredited school of Nursing and possess a current Practical Nurse license in good standing from the Commonwealth of Pennsylvania.
- Demonstrated management, communication, and observation skills with an interest in geriatrics.
- Working knowledge of computers and Microsoft Word required.
