Job SummaryWe are looking for a dedicated and experienced nurse to join our team and provide high-quality care to patients who require specialized nursing services. As a private duty nurse, you will work one-on-one with patients to develop and implement individualized care plans that meet their unique needs and goals.
Responsibilities- Provide skilled nursing care to patients in their homes, including medication management, wound care, and vital sign monitoring
- Develop and implement individualized care plans that meet the unique needs and goals of each patient
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
- Document patient progress and outcomes in accordance with company policies and procedures
To be successful in this role, you will need:
- A current LPN license in the state of New York
- Experience working with medically fragile patients, preferably in a home health or private duty setting
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to work independently and as part of a team, with a focus on providing high-quality patient care
