The Clinical Manager will oversee all patient/client care services and personnel, ensuring quality of patient/client care in the home. The Clinical Manager will coordinate care with the interdisciplinary team, patient/client, family, and referring agency.
Key Responsibilities:- Coordinate patient/client care, integrating care provided by Agency staff, including contractual providers.
- Ensure communication with all physicians involved in patient's/client's plan of care and other healthcare practitioners.
- Integrate orders from all relevant physicians involved into the plan of care and ensure the orders are approved by the responsible physician.
- Coordinate referrals and participate in activities relevant to professional services, including development of qualifications and resource recommendations.
- Assure that patient/client needs are continually assessed and reassessed as necessary.
- Develop, implement, and update individualized plans of care in partnership with the patient/client, representative (if any), and caregiver(s).
- Ensure provision of services as ordered by the physician, including patient/client and caregiver education and patient/client, caregiver, and family counseling.
- Assume responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations.
- Collaborate on development and implementation of policies and procedures guiding and supporting the provision of care.
- Monitor and participate in the QAPI program and ensure appropriate corrective measures are performed.
- Promote compliance with all fiscal intermediary and/or other third-party payors through education, coaching, and other assistance as necessary.
- Monitor systems and identify problem areas to report to the Administrator both verbally and through written reports.
- Meet mandatory continuing education requirements of the Agency and licensing board.
- Promote and educate regarding the concepts of infection control and standard precautions in coordinating or performing patient/client care activities to prevent contamination and transmission of disease.
- Use effective interpersonal relations and communication skills to facilitate desirable outcomes.
- Participate in Agency-sponsored in-service trainings.
- Ensure that care offered by the Agency, including any branches, is consistently available to all patients/clients.
