Position Summary
We are in search of a Bilingual Nursing Care Coordinator who will be instrumental in providing specialized primary care and nursing services to our Spanish-speaking clientele. This role is essential for fostering a supportive environment for clients navigating their recovery journey. If you possess a robust nursing background and are fluent in Spanish, this position may align perfectly with your professional aspirations.
Key Responsibilities
- Provide integrated primary care and mental health services, focusing on the needs of Spanish-speaking clients.
- Collaborate with the healthcare team to create and execute personalized treatment plans.
- Conduct comprehensive medical evaluations, crisis interventions, and health screenings, prioritizing clients with high-risk conditions.
- Educate clients on medication management, chronic disease prevention, and healthy lifestyle choices.
- Coordinate care with external healthcare providers and maintain accurate documentation within Health Information Systems.
- Advocate for clients' access to essential benefits and services, including Medicaid and third-party insurance.
- Engage in community outreach efforts to cultivate strong, culturally sensitive relationships with clients and their families.
Qualifications
- Licensed LPN or RN in Oklahoma with experience in community-based mental health settings.
- Fluency in Spanish, with excellent verbal and written communication skills.
- Proven ability to establish rapport with clients, colleagues, and external partners.
- Commitment to recovery-focused, trauma-informed care principles.
- Compliance with all certification and training requirements, including Therapeutic Options and First Aid/CPR.
