Kidney Disease Care Specialist

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Kidney Disease Care Specialist. This role will be responsible for coordinating care for patients with end-stage renal disease, promoting home dialysis modalities, and educating patients on self-management.

Responsibilities- Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs
- Create and implement care plans to address identified needs and improve patient health
- C coordinate care by serving as an advocate and resource for patients, their families, and healthcare providers
- Facilitate care transitions to minimize avoidable readmissions
- Provide education and support to patients on self-management and kidney disease management
- 1+ years of nursing experience in case management or care management
- Current, unrestricted compact Registered Nurse license
- BLS certification from a licensed AHA or American Red Cross training facility or provider
- Core values consistent with a patient-centered approach to care
