About the Role

LHC Group is seeking a skilled Physical Therapist to join our team in Senior Living. As a Physical Therapist, you will play a vital role in helping patients achieve their rehabilitation goals.

Responsibilities
  • Evaluate patients to determine their rehabilitation potential and identify their needs.
  • Develop and implement individualized treatment plans of care, setting goals to be used in the team plan of care.
  • Utilize equipment in a safe and proper manner, maintaining equipment inspection, maintenance, and cleaning logs.
  • Follow medical regimens developed for patients, adhering to precautions such as THR, TKR, Cardiac, and Wound.
  • Complete discharge summaries on each patient on their final visit to outpatient PT.
Requirements
  • Current Physical Therapist licensure in the state of practice.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
