About the Role

We are seeking a skilled Social Worker to join our team at RHI DVA Renal Healthcare, Inc. As a vital member of our dialysis care team, you will play a critical role in supporting patients with End Stage Renal Disease.

Key Responsibilities
  • Build meaningful relationships with patients and their families in an intimate outpatient setting.
  • Collaborate with our multidisciplinary team to provide comprehensive care and support.
  • Guide patients in managing the physical, mental, emotional, and financial demands of dialysis.

Requirements

To be successful in this role, you will need:

  • A Master's Degree in Social Work (MSW) with a specialization in clinical practice.
  • Meets all state required regulations to practice in the dialysis setting.
  • Obtained their Clinical License (e.g., LCSW, LICSW), meeting all licensure requirements for their state.
  • Proficient in all Experienced level Social Worker duties.
  • Intermediate computer skills.
  • Demonstrated knowledge of government and private insurance programs.
  • Excellent interpersonal and communication skills.
