Job Summary

CVS Health is seeking a compassionate and experienced Peer Support Specialist to join our team. As a Foster Care Peer Support Specialist, you will play a vital role in guiding, coaching, and mentoring individuals in the foster care system. Your personal experience with trauma and/or foster care will enable you to provide empathetic support and connect members with essential resources.

Key Responsibilities
  • Utilize your lived experience to educate healthcare providers on cultural, ethnic, and religious preferences.
  • Collaborate with care managers, care coordinators, and members to develop personalized action plans.
  • Provide coping strategies, wellness tools, and resources to support members' unique needs.
  • Bridge the gap between the member, child welfare, and healthcare systems through outreach and education.
Requirements
  • Lived experience in foster care and/or a personal background of trauma.
  • Knowledge of local community supports and services.
  • Ability to work collaboratively in a team and manage multiple priorities.
  • Experience providing care to socially and medically complex patients.
Preferred Qualifications
  • Mental Health First Aid, Trauma-Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices.
  • Exceptional communication, organizational, and interpersonal skills.
