We are seeking a skilled Physician to join our team at CenterWell Senior Primary Care. As a Physician, you will play a vital role in providing high-quality, patient-centered care to our seniors.
Key Responsibilities- Provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
- Work collaboratively with our integrated care teams, including physicians, nurses, behavioral health specialists, and more, to provide compassionate, personalized care.
- Focus on outpatient medicine, continuity of care, health maintenance, and disease prevention.
- Keep accurate medical histories and medical records.
- Refer patients to specialists as needed.
- Make decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction.
- Bachelor's degree.
- 5 or more years of technical experience.
- Licensure requirements of the state of jurisdiction.
- Graduate of accredited MD or DO program of accredited university.
- Prefer Internal Medicine specialty.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
