About the Role

We are seeking a dedicated and compassionate LVN/RN to join our team at Magnolia Place Skilled Nurse. As a member of our healthcare team, you will play a vital role in providing high-quality patient care and support to our residents.

Key Responsibilities
  • Facilitate patient care as directed by the attending physician and supervise nursing activities.
  • Assist with pre-screening, room assignments, and resident discharges.
  • Promote and maintain high standards of care quality.
  • Ensure a safe and comfortable environment by adhering to safety protocols.
  • Conduct interviews, evaluations, and selection of new staff members.
  • Communicate any new or unusual findings to relevant parties.
  • Uphold confidentiality while fostering positive relationships with residents, team members, and visitors.
  • Monitor and document patient health and progress and implement care plans.
  • Provide education and support to residents and their families about care procedures and health management.
  • Participate in quality improvement initiatives and contribute to a collaborative team environment.
Requirements
  • Graduate of an accredited LVN/RN program with a current state license.
  • Minimum of 2 years of nursing experience in a skilled nursing facility (preferred).
  • 1 year of experience in a secure/memory care unit.
  • Proficient in care planning processes and electronic medical records.
  • Strong attention to detail, documentation, and resident assessment skills.
