UMass Memorial Health Care is seeking a skilled Licensed Practical Nurse to join our team as a Caregiver. In this role, you will provide high-quality patient care and coordinate referrals as prescribed by healthcare providers.
Key Responsibilities- Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, medication administration and documentation.
- Provide appropriate healthcare and coordinate referrals as prescribed by providers.
- Maintain documentation required for all primary care clients receiving in-home nursing services.
- Promote education and adherence to prescribed treatment protocols.
- Communicate with healthcare facilities when clients are admitted and plans for discharge to ensure continuity of care.
- Facilitate wellness education to individuals and offer groups with health and wellness focus.
- Record all client information within electronic medical record system daily, received by outside healthcare agencies.
- Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
- Must be able to pass a CORI background check.
- Preferred: 2-4 years' experience in medical nursing.
- Must be able to be in a standing position for extended periods of time.
- Must be able to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms).
- Must be able to lift and move objects up to 50 pounds for various needs.
