Job Title: Licensed Practical Nurse

UMass Memorial Health is seeking a skilled Licensed Practical Nurse to join our team. As a key member of our healthcare team, you will provide high-quality patient care and support to our patients and their families.

Job Summary:

The Licensed Practical Nurse will be responsible for assessing the physical and psychosocial status of patients, administering medications, and providing education and support to patients and their families. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Assess patients' physical and psychosocial status, including medical history, current health status, and social and environmental factors.
  • Administer medications and treatments as prescribed by physicians and other healthcare professionals.
  • Provide education and support to patients and their families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices.
  • Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Document patient information and progress in the electronic medical record system.
Requirements:
  • Graduate of an accredited school of nursing with current licensure as a Licensed Practical Nurse in the state of Massachusetts.
  • Minimum 2 years' experience in medical nursing, with a focus on substance abuse and mental health.
  • Knowledge of medical, substance abuse, and mental health nursing principles and practices.
  • Ability to communicate effectively with patients, families, and healthcare team members.
