Position Overview:
This full-time role at our bustling Greeley center involves collaboration with fellowship-trained clinicians, making it an ideal opportunity for both recent graduates and seasoned therapists.

Compensation:
Salaries range from $89,000 to $104,000 annually, depending on experience, plus a potential $10,000 annual KPI bonus.

Work Schedule:
Monday to Friday, from 9:00 AM to 6:00 PM, with the option of four 10-hour shifts per week.

Key Responsibilities:
  • Assess and evaluate patients' physical therapy requirements, develop tailored treatment plans and objectives, and execute these plans with physician endorsement.
  • Complete all necessary documentation accurately and promptly, including initial evaluations, daily treatment notes, staffing reports, discharge summaries, timely reassessments, patient education documentation, billing-related records, and insurance justification letters.
  • Provide educational services to patients, both verbally and in writing, ensuring all activities and patient responses are documented in their charts.
  • Supervise the Physical Therapy Assistant in delivering therapy services as required.
  • Engage in treatment plan reviews, patient case discussions, assessments, and consultations with physicians.
  • Adhere to company policies and procedures, maintaining strict patient confidentiality.
  • Assist the Medical Director with tasks related to Utilization Review and Quality Assurance as directed.
  • Perform additional duties as assigned.

Qualifications:
A Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited institution is required. A valid license (in good standing) or diploma to practice physical therapy in Colorado is mandatory, along with current CPR certification.
