Job Summary:
Summit Home Care is seeking a skilled Registered Nurse (RN) to perform in-home UAS assessments in the Bronx. As a key member of our team, you will be responsible for conducting comprehensive assessments, reviewing medical records, and consulting with healthcare providers to support assessment outcomes.
Responsibilities:
- Assess eligibility of Medicaid members in need of managed long-term care using a comprehensive web-based system.
- Conduct initial and annual assessments remotely, according to state guidelines, policies, and protocols.
- Consult with healthcare providers and review medical records and medications to support assessment outcomes.
- Review and revise assessments as necessary based on company policy.
Requirements:
- New York State Registered Nursing (RN) license.
- New York State Drivers License or New York State Identification Card (if living out of NYS State).
- Must be a resident of the United States.
