As a Home Health Physical Therapist with CenterWell Primary Care, you will play a vital role in helping patients manage chronic conditions and recover from injuries or illnesses. Our team of dedicated healthcare professionals is committed to providing personalized, comprehensive care that puts patients at the center.
Key Responsibilities:
- Plan and administer prescribed physical therapy treatment and training for patients to achieve optimal physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and develop treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physician plans of treatment.
- Confer with physicians and clinical team members to obtain additional patient information and develop treatment plans.
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, supervise and evaluate their performance.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Report patients' progress to physicians, clinical managers, staff, patients, and families.
Requirements:
- Degree from an accredited Physical Therapy Program.
- Minimum of one year physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation.
