Job Summary:We are seeking a skilled Post-Acute Care Specialist to join our team at CarePartners Health Services in Asheville, North Carolina. As a key member of our interdisciplinary care team, you will work closely with patients, families, and healthcare professionals to provide high-quality, patient-centered care.
About the Role:The ideal candidate will have a Bachelor's degree in Nursing or a related field and a minimum of 5 years of recent clinical nursing experience in a relevant practice area. You will be responsible for assessing patient needs, developing personalized care plans, and coordinating care across the continuum of hospitalization.
