Job Summary

Covenant Health is seeking a highly skilled Nurse Practitioner to join our Quality Management team. As a key member of our team, you will provide primary care to patients under the supervision of a physician preceptor, following established medical protocols.

Responsibilities
  • Maintains established departmental policies and procedures, objectives, quality assurance program, and safety standards.
  • Provides direct patient care, evaluates outcomes, consults with other specialists as required, and adjusts processes as indicated to ensure optimal patient care.
  • Collaborates with physician preceptor to formulate and review medical protocols.
  • Obtains complete medical history of the patient, including both inpatient and outpatient settings, to ensure new consults for the structural program are completed in a timely fashion.
  • Identifies health hazards in the patient's environment as well as patient risk factors, and considers cultural patterns, economic status, and patient resources in the plan of care.
  • Performs appropriate physical exam as indicated, obtains appropriate laboratory specimens, and analyzes objective and subjective data for appropriate diagnosis and treatment.
  • Assures treatment and/or plan of care is consistent with established protocols, monitors and ensures accuracy of recording on patient medical records, and maintains all required reports, records, statistics, etc.
  • Documents all pertinent information accurately, legibly, and in a timely manner, and follows Medicare guidelines for accuracy in CPT and ICD coding.
  • Applies the fundamentals of AIDET when communicating with patients, refers patients to appropriate services which exceed the scope of practice of the practitioner, and provides health promotion counseling and develops patient teaching information.
  • Serves as a resource person for other staff personnel, leads new initiatives and/or educational programs for both structural and valve pre-operative and post-operative care for both existing and new therapies for our patients, and is responsible for periodic review of policy and procedures.
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of the latest trends in the field of expertise, and attends meetings as required and participates on committees as directed.
  • Performs other duties as assigned or requested, including assisting with preparation for structural case reviews, acting as a liaison between cardiology, cardiothoracic surgery, anesthesia, and other specialties involved in the care of structural patients, and serving as a liaison for structural and valve leadership with the hospital and nursing administration team.
Qualifications
  • Minimum Education: None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by issuing authority.
  • Minimum Experience: One (1) year experience.
  • Nurse Practitioner licensed in the state of TN.
  • Minimal travel may be required.
