About the Role:

We are seeking an experienced Physical Therapist to join our team at Rehab Without Walls Neuro Rehabilitation. As a Physical Therapist in our Home and Community Program, you will have the opportunity to make a meaningful impact in the lives of our patients by providing functional rehabilitation in a real-life setting.

Key Responsibilities:

  • Perform evaluations and develop treatment plans to help patients regain functional capabilities
  • Regularly re-assess effectiveness of treatment plans and communicate with the treatment team, physician, and family members
  • Perform discharge evaluations and provide recommendations for home and community safety
  • Communicate effectively with patients, families, and healthcare professionals to ensure collaboration and coordination of care

Requirements:

  • Minimum of bachelor's degree in Physical Therapy from an accredited program
  • Current, unrestricted license as a Physical Therapist in the state of practice
  • Current Basic Life Support or Cardiopulmonary Resuscitation certification
  • Minimum of one year's work experience as a Physical Therapist preferred
