Skies Healthcare and Rehab Center in Albuquerque, NM is seeking a dedicated and compassionate Social Services Assistant to support our social service staff in providing exceptional care to our patients and residents.

Key Responsibilities:
  • Collaborate with patients and their families to provide support and resource information
  • Respond to issues identified by patients and their families
  • Facilitate patient transfer within the nursing center to ensure a seamless transition
  • Document patient information and progress
Requirements:
  • Associate degree in human services or related field
  • Minimum of two years' experience in long-term care
  • Certification required by state regulations
