We are seeking a proficient Case Manager to improve patient management quality, enhance patient care, and promote overall cost-effectiveness.

The Case Manager will oversee the complete patient care cycle, including scheduling clinical appointments, connecting patients with community resources, following up after hospital discharges and clinical visits, and addressing any social determinants of health (SDOH) needs.

Responsibilities include assessing, planning, implementing, monitoring, and evaluating actions necessary to meet clients' health and human services requirements.


Key Responsibilities

  • Guide patients through the healthcare system as an advocate and navigator.
  • Engage in quality improvement initiatives (HEDIS, Value-based care, PCMH).
  • Identify care gaps and coordinate services to address them.
  • Ensure continuity of care with external healthcare organizations and facilities, including hospital admissions, discharges, and referrals.
  • Support continuity of care with patients and families following hospital admissions, discharges, and emergency room visits.
  • Co-manage high-risk patient care, including those with multiple comorbidities or at high risk for readmission, in collaboration with the CCM team.
  • Review all positive SDOH screenings and provide relevant community resources and referrals.
  • Maintain clear and timely communication through documentation and team meetings regarding patient status and care plans.
  • Participate in data collection, health outcomes reporting, clinical audits, and evaluations related to Patient-Centered Medical Home initiatives.
  • Contribute to clinical care processes, resource utilization, and the development of new clinical tools and procedures to enhance patient care.

Qualifications and Experience

  • Bachelor's degree in Social Work or an active LPN license.
  • 1-2 years of relevant experience, including 1-2 years in Case Management.
  • Prior office experience preferred, especially with various EMRs (experience with Athena is a plus).
  • Experience in communicating with insurance companies to obtain patient data and resources.
  • Hands-on experience utilizing patient care plans in direct coordination with medical providers.
  • Ability to manage a case load of approximately 300 patients per month.
  • Strong communication skills to educate patients and their families.
  • Intermediate computer skills, particularly in Microsoft products (e.g., Excel and Word).
  • Self-motivated, reliable, and detail-oriented.
  • Ability to maintain confidentiality and exercise discretion in escalating issues to providers.
  • Excellent interpersonal and customer service skills, functioning as a positive team member.
  • Capacity to sit for extended periods and use a computer keyboard.
  • Able to handle stress and conflict appropriately.
