Role Summary: Under the guidance of the Director of Nursing, the MDS Coordinator/RN plays a pivotal role in managing the MDS/Resident Assessment Instrument. This position involves overseeing the ongoing education of staff involved in MDS preparation and ensuring the accurate electronic submission of MDS data to the relevant health authorities. The MDS Coordinator is instrumental in facilitating effective communication among the interdisciplinary team and contributes to resident assessments, care planning, and evaluation processes in accordance with established policies and the State Nurse Practice Act.

Key Responsibilities:

  • Schedule and execute MDS and RAP assessments in compliance with State and Federal regulations.
  • Input and manage data within the healthcare information system.
  • Provide training and support to interdisciplinary team members regarding their MDS responsibilities.
  • Conduct resident interviews as necessary to complete MDS documentation.
  • Transmit MDS data to the appropriate health department.
  • Participate in Aims testing and collaborate on medical rounds, orders, and consultations.
  • Engage in Fall and Psychosocial committees and contribute to interdisciplinary care conferences.
  • Develop initial nursing care plans and oversee their implementation.
  • Screen potential admissions for the Nursing Facility.
  • Utilize the nursing process to create and execute comprehensive care plans that optimize patient outcomes.
  • Address the educational needs of patients, families, and colleagues through effective teaching strategies.
  • Delegate nursing tasks based on staff qualifications and patient care requirements.

Qualifications:

Education: A nursing degree from an accredited institution.

Licensure: Must hold a valid registered nursing license in the state of ND or a compact state. Graduate Nurses (G.N.) must work under the supervision of a registered nurse while their practice authorization is active.

Experience: Prior basic MDS training is preferred.

Compensation:

$36.92 per hour
