Key Responsibilities

ROLE SUMMARY: Under the guidance of the Director of Nursing, the MDS Coordinator/RN plays a crucial role in managing the MDS/Resident Assessment Instrument. This position is essential for ensuring the accurate and timely electronic submission of MDS data to the State Department of Health. The coordinator is expected to demonstrate proficiency in completing the MDS/Resident Assessment Instrument and to facilitate effective communication among the interdisciplinary team. Additionally, the role involves assisting in resident assessments, care planning, and evaluating care plans in accordance with established policies and the State Nurse Practice Act.

All duties and responsibilities outlined herein will be executed in compliance with the Medical Center's policies, procedures, and guidelines.

PRIMARY DUTIES:

  • Schedule and execute MDS and RAP assessments in line with State and Federal regulations.
  • Input relevant data into the electronic health record system.
  • Provide assistance and training to interdisciplinary team members in completing their MDS sections.
  • Conduct resident interviews as necessary for MDS completion.
  • Ensure the transmission of MDS data to the State Department of Health.
  • Participate in Aims testing.
  • Conduct rounds with physicians, manage orders, and coordinate consultations.
  • Engage in Fall and Psychosocial committees.
  • Contribute to interdisciplinary care conferences.
  • Develop and implement initial nursing care plans.
  • Screen potential admissions for the Nursing Facility.
  • Utilize the nursing process to create and execute comprehensive care plans that promote optimal patient outcomes.
  • Address the educational needs of patients, families, and colleagues using effective teaching strategies.
  • Delegate nursing responsibilities based on staff qualifications and patient care requirements.
Qualifications

Education: A nursing degree from an accredited nursing program.

Licensure: Must hold a valid registered nursing license in the state of ND or in a compact state. Graduate Nurses (G.N.) must work under the supervision of a registered nurse while their practice authorization is valid.

Experience: Basic training in MDS processes is preferred.
