We are seeking a compassionate and skilled CNA Hospice Home Care Specialist to join our team at CarePartners Living LLC. As a CNA Hospice Home Care Specialist, you will play a vital role in providing high-quality, patient-centered care to our patients.

Responsibilities
  • Deliver high-quality, patient-centered care by performing a variety of delegated basic patient care services related to common nursing functions and activities of daily living.
  • Assist patients with activities of daily living, including eating and drinking, bathing, shaving, oral care, grooming, and elimination.
  • Change bed linens as needed.
  • Assist patients with mobility by regular turning and positioning, dangling, and ambulating patients.
  • Assist with the use of walkers, crutches, canes, and wheelchairs.
  • Transfer patients to and from other units, departments, or entities.
  • Assist with discharges.
  • Engage in evidence-based best practices that enhance the patient experience, including SAFETY rounding, communication fundamentals/AIDET, Teach-Back, Narrating Care, etc.
Requirements
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Certified Nursing Assistant (CNA).
  • Drivers License.
  • High School Graduate/GED.
