Job Title: Hospice Nurse Practitioner PRN

At Aveanna Healthcare LLC, we are seeking a skilled and compassionate Hospice Nurse Practitioner PRN to join our team. As a Hospice Nurse Practitioner, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities:

  • Perform face-to-face encounters with hospice patients to determine continuing eligibility for care.
  • Gather and document clinical findings to support patient care decisions.
  • Communicate effectively with interdisciplinary teams and patients' families to ensure seamless care coordination.
  • Meet productivity expectations, completing 10-20 visits per month.

Requirements:

  • Current License as Nurse Practitioner in the State of Tennessee.
  • Minimum of one (1) year medical/surgical experience, with hospice/home health experience preferred.
  • Acceptance and support of the hospice concept of care.
  • Excellent assessment and problem-solving skills, with strong communication and time management abilities.

Preferred Qualifications:

  • Current, valid Tennessee Driver's License.
  • Proof of automobile insurance coverage.
