Job Summary

We are seeking a highly motivated and compassionate Direct Support Professional to join our team at WellLife Network. As a Direct Support Professional, you will play a vital role in supporting individuals with intellectual/developmental disabilities and mental illness in achieving their goals and living independently.

Key Responsibilities
  • Develop and implement individualized plans to support individuals in achieving their goals and living independently.
  • Provide training and support in daily living skills, including budgeting, grooming, nutritional planning, and use of public transportation.
  • Monitor progress and provide feedback to individuals and their support teams.
  • Collaborate with interdisciplinary teams to develop and implement treatment plans.
  • Provide crisis intervention and incident reporting as needed.
  • Maintain accurate and confidential records of individual progress and treatment plans.
Requirements
  • High School Diploma or equivalency diploma required.
  • Experience working with adults with behavioral health or other disabilities preferred.
