Job Summary

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address their unique requirements.
  • Deliver skilled nursing interventions, including wound care, medication management, and education on disease management and prevention.
  • Collaborate with the RN Supervisor to ensure seamless care transitions and coordinate with other healthcare professionals as needed.
  • Maintain accurate and detailed records of patient interactions, including visit notes and progress updates.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
