Job Title: LPN Home Health Full Time

About the Role:

We are seeking a compassionate and skilled LPN to provide professional nursing services to our patients. As an LPN, you will work under the direction of a Registered Nurse (RN) to deliver care that is tailored to each patient's unique needs and goals.

Responsibilities:
  • Provide skilled nursing interventions to patients in their homes, including wound care, medication management, and education.
  • Collaborate with the RN to develop and implement individualized care plans that meet the patient's physical, emotional, and social needs.
  • Conduct thorough assessments of patients' conditions and develop plans to address any identified needs or concerns.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
  • Graduate of an accredited Licensed Practical Nursing program.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
