- Facilitate skills groups and provide individualized support to clients, promoting positive behaviors and outcomes.
- Conduct in-home sessions with clients and their families, providing education and support to enhance their well-being.
- Ensure accurate and timely documentation of client services and progress, adhering to PATH Integrated Health's standards and compliance requirements.
- High School Diploma or equivalent required; Bachelor's degree preferred.
- Experience working with at-risk youth and their families, preferably in a mental health setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, colleagues, and community partners.
- Ability to work in a fast-paced environment, managing multiple priorities and deadlines.
- Proficiency in electronic health records and other software applications used by PATH Integrated Health.
- Competitive hourly rate: $15-$20 based on experience.
