Key Responsibilities:

The MDS Care Coordinator is responsible for managing the RAI process, conducting assessments, and coordinating care plans for assigned residents.


Specific duties include:
  • Completing the MDS, CAAs, and care plans within required timelines.
  • Performing resident assessments through physical evaluations, interviews, and chart reviews.
  • Collaborating with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activity personnel to address resident care needs.
  • Reviewing information from hospitals and external agencies to inform assessments and care planning.
  • Coordinating and identifying significant changes in MDS.
  • Preparing for PPS meetings and maintaining MDS assessments in accordance with Medicare schedules.
  • Staying updated with the requirements set by the American Association of Nursing Assessment Coordinators (AANAC).

Qualifications:
  • Registered Nurse (RN)
  • AANC certification is advantageous; RAC-CT preferred.
  • Familiarity with the Resident Assessment Instrument (RAI) process and the Prospective Payment System (PPS) is highly desirable.
  • Previous experience as an MDS Nurse.
