Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team at Harbor Retirement Associates. As a key member of our healthcare team, you will provide direct and individualized nursing care to our residents, ensuring their safety and well-being.

Key Responsibilities
  • Provide high-quality nursing care to residents, adhering to established standards and protocols.
  • Implement care plans developed by physicians and the Director of Resident Care, ensuring seamless execution.
  • Document all care provided accurately and in a timely manner, maintaining confidentiality and resident rights.
  • Administer medications as prescribed, monitoring for adverse reactions and reporting to physicians as necessary.
  • Prepare incident/accident reports, notifying physicians and family members as required.
  • Maintain a safe and healthy working environment, adhering to clinical standards and procedures.
  • Assist residents in learning self-care activities, promoting independence and dignity.
  • Organize and monitor resources necessary for resident care, ensuring efficient and effective delivery.
  • Recognize emergencies and life-threatening situations, initiating appropriate action.
Requirements
  • Graduation from an accredited LPN/LVN program and current registration in the state of the community.
  • Valid Continuing Education credits as required by the state.
  • Training in Alzheimer and Dementia with a focus on Validation Therapy.
  • Minimum one year of nursing experience in a healthcare facility, with long-term care and geriatric nursing experience preferred.
  • Ability to communicate effectively with all levels of management, associates, residents, family members, and outside contacts.
  • Experience with Microsoft Office and Outlook software is a plus.
