About the Role

As a Care Coordinator, you'll play a vital role in ensuring that individuals receive the services they need in a supportive, effective, efficient, timely, and cost-effective manner.

This is a field-based position with a home-based office, requiring local travel up to 75% and mileage reimbursement at the current government rate. You'll work closely with individuals, families, and healthcare teams to assess, plan, and implement care strategies that meet the unique needs of each person.

Key Responsibilities
  • Assess and develop individualized care plans that address social determinants of health and integrate into community
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously
  • Advocate for individuals and families to ensure their needs and choices are fully represented and supported by the healthcare team
  • Conduct home visits and in-person visits to assess and support individuals in their care settings

Requirements
  • Bachelor's Degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services area
  • 1+ years of experience working with individuals with long-term care needs
  • 1+ years of experience working within the community health setting in a healthcare role
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications
  • Ability to travel 75% of the time and possess a valid driver's license, current automobile insurance, and access to reliable transportation
Preferred Qualifications
  • Licensed Social Worker or clinical degree
  • 1+ years of experience working with individuals receiving services on one of the home and community-based waivers in KS
  • Background in managing populations with complex medical or behavioral needs
  • Experience with electronic charting and arranging community resources
Soft Skills
  • Strong communication and customer service skills, both in person and via phone
  • Ability to work independently and maintain good judgment and accountability
  • Demonstrated ability to work well with others, strong organizational and time management skills, and ability to multi-task and prioritize tasks to meet deadlines
