Job Opportunity

At American Veterans Hospice, we are seeking a compassionate and skilled Registered Nurse (RN) to join our team. As a Hospice RN, you will provide high-quality patient care and support to individuals and their families in San Bernardino County.

Responsibilities:

  • Conduct thorough patient assessments to determine hospice needs and develop individualized care plans
  • Provide direct skilled bedside nursing care, including medication administration and wound care
  • Evaluate and maintain a safe environment for patients
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Participate in ongoing education and training to enhance skills and knowledge

Qualifications:

  • Experience in hospice care visits and a valid State license
  • Excellent communication and organizational skills
  • Proficiency in computer and smartphone use for electronic notes
  • Knowledge of HospiceMD software
  • Bilingual preferred
