About the Role
This position will coordinate kidney transplant care for patients of all ages, ensuring seamless coordination of care from diagnosis through transplantation and beyond. As a Senior Transplant Nurse Program Coordinator, you will oversee program activities and resources within clinical specialty or department, making recommendations and acting as a resource for nursing staff related to patient care and clinical standards, policies, and procedures.
Responsibilities
- Care Plan Development: Develop and document comprehensive care plans and therapy programs tailored to individual patient needs.
- Program Oversight: Oversee and ensure plan of care therapy programs are implemented as indicated, ensuring necessary resources and equipment are obtained as ordered.
- Nursing Staff Support: Provide guidance and support to nursing staff on patient care and clinical standards, policies, and procedures.
- Supervision and Direction: May supervise and direct nursing staff, promoting a culture of excellence and teamwork.
Requirements
To be successful in this role, you must have:
- A current Registered Nurse licensure from the Indiana Board of Nursing.
- A Bachelor's degree in Nursing and three years of direct patient care experience.
- Prior experience in transplant, dialysis, or renal patient care is preferred.
